Classification of Mental Retardation

Intellectual and Developmental Disabilities or mental retardation can be categorized under four groups as enumerated below.

  • Mild Mental Retardation: Approximate IQ range of 50 to 69 ( in adults, mental age from 9 to 12 years). This might lead to learning difficulties in school. But, many adults under this type are able to work properly and maintain a healthy social relationship. These adults are generally feeble-minded. They can learn practical life skills to function properly in ordinary life with minimum support.

  • Moderate Mental Retardation: Approximate IQ ranges between 35 to 49 (in adults, mental age from 6 to under 9 years). This may result in developmental delays in children but these adults can learn self-care, academic, and communication skills. However, they do need support to live and work in society.

  • Severe Mental Retardation: Approximate IQ range between 20 to 34 (in adults, mental age from 3 to under 6 years. People with severe mental retardation need constant support for basic or day to day tasks. However, they do need supervision in different social settings and family care for living in a group home.

  • Profound Mental Retardation: Approximate IQ under 20 (in adults, mental age below 3 years). This might result in facing difficulties in self-care, mobility, and communication. They have congenital syndromes and need supervision with self-care activities. Moreover, they have very limited capability to communicate and physical limitations.

Mental Retardation Characteristics

Mental retardation centers around impairment of general mental abilities that impacts adaptive functioning in three domains. These domains are used to determine how well an individual works with everyday tasks:

  • Conceptual Domain: Includes skills in reading, writing, language, mathematics, reasoning, memory, and knowledge.
  • Social Domain:This comprises empathy, interpersonal communication skills, social judgment, etc.
  • Practical Domain: It includes self-management skills in several areas like personal care, money management, job responsibilities, etc.

Treatment Categories for Mental Retardation

The treatment for mental retardation falls within three categories

  1. Treatments that address or minimize the underlying causes of mental retardness like restricting phenylalanine patients’ diet who have phenylketonuria.
  2. Treatments of comorbid mental and physical disorders in order to improve the patients’ functions. For instance, pharmacologic treatments of behavioral disorders for children with fragile X syndrome.
  3. Behavioral and cognitive interventions along with psychosocial support, special education, etc.

DSM-5 Approach to Diagnosing Intellectual Disability

Here is the DSM-5 (Diagnostic and Statistical Manual of Mental Disorders) approach to diagnosing intellectual disability or mental retardness:

  • Clinical Assessment and Standardized Testing: DSM-5 emphasis on using both clinical assessment and standardized intelligence tests for diagnosis.
  • Severity Based on Adaptive Functioning: Severity of impairment should be determined by adaptive functioning and not just IQ scores.
  • IQ Scores in Diagnostic Criteria: IQ scores are excluded from the primary diagnostic criteria but must be included in the descriptive text. This eliminates overemphasis on IQ scores alone, ensuring comprehensive evaluation of functioning.
  • Inclusion of IQ Scores:IQ scores or similar standardized test scores must be part of the assessment. Intellectual disability is generally defined as two standard deviations below the population average (IQ score of about 70 or below).
  • Assessment Across Three Domains: The intelligence assessment is done under three domains like conceptual, social, and practical. This helps to ensure diagnosis is based on the impact of deficits on everyday functioning.
